Information Privacy Statement (Public)

HumanisRx is committed to protecting the privacy and security of any personal information obtained because of your client relationship with us.

Why we collect, use and share personal information

Our staff collects, uses, and shares personal information for the following purposes:

  • To provide patients or benefit plan members with information regarding medication and health information related to their condition, medication reviews, care plans, drug interactions, opportunities to reduce risks from therapy, or recommendations to optimize therapy, and other healthcare services as appropriate for their needs;
  • To work with patients’ or benefit plan members’ healthcare providers to optimize their care. For example, discussing medication issues with prescribing physicians, making recommendations to modify medication or medication doses, monitoring care and assisting with any additional medication needs;
  • To provide patients or benefit plan members with information, additional program recommendations and/or referrals that may assist in their health conditions;
  • To access Provincial Health Record(s), in applicable provinces and with the patient or benefit plan member’s consent, to provide the healthcare services above; 
  • To help improve the quality of services through understanding our client’s needs, conducting market research and investigating new business processes;
  • To create an electronic patient and/or client record;
  • To fulfill our professional, legal, or regulatory obligations.

How information is collected

We collect information directly from patients or benefit plan members that select to share with us when they contact one of our staff or use our services. We may also collect information indirectly through other people involved in a patient’s or benefit plan member’s healthcare. For example, we may collect information:

  • From physicians, other prescribers, community pharmacist or healthcare providers when clarifying medication needs;
  • From benefits providers or a disability management company that is providing services to patients or plan members, in order to assist in managing their medication therapy program;
  • From patient’s or benefit plan member’s electronic health records
  • From patient’s or benefit plan member’s authorized representative or substitute decision-maker;
  • From Provincial Health Record(s), if applicable;
  • From drug claims provided via the patient or benefit plan member’s third- party administrator; and
  • From Microsoft Bookings or other appointment scheduling app, if applicable

How information is shared and disclosed

When we provide a patient or benefit plan member with healthcare services, we may share their personal health information with their healthcare providers when it is necessary for their healthcare. We do not share personal health information without a patient’s or plan member’s consent for other purposes unless we are legally required to do so. For example, personal information may be disclosed to respond to a court order or under a specific statute (e.g. the Coroner’s Act or Adult Guardianship Act).

We use service providers and subcontractors who assist us in our business. These service providers and subcontractors may have access to personal information while they perform specific duties for us. They are not permitted to use personal information except as authorized by us.

We may share personal information in connection with the review of and the completion of business transactions involving all or part of our business. Examples of business transactions include mergers, sales, financings, reorganizations or other transactions involving our assets. We will require that any parties involved in these transactions who receive personal information only use that information in connection with the transaction and for the purposes for which personal information was collected.

How HumanisRx ensures that personal information is secure

We use electronic and paper records to manage and store information efficiently, and to improve the quality of care we deliver.

We record personal information in our pharmacy information systems on premises and/or in an offsite data center, and use automated backups to same site data center in order to ensure security and availability of information. All locations are treated as secure and accessible only by authorized personnel.

We use a variety of safeguards to protect your electronic information including encryption, passwords, and access audits. Best industry practices and protocols for security are implemented and maintained in compliance with Privacy Legislation.

Patients and benefit plan members can make a request for personal information HumanisRx has collected

An individual may request to view or correct personal information by means of written request to the location at which they received service.
